Call the office
You should be greeted warmly and your needs should be addressed with care and compassion. Since a considerable portion of your interaction with the surgeon will occur through the office team, they should be helpful, considerate, caring and genuinely willing to help.
Learn about your surgeon
Research your surgeon’s credentials. Find out where and when he/she went to school. What associations is your surgeon active in? What are his/her areas of clinical interest?
Can you get an appointment?
The surgeon’s team should be accommodating and focused on providing you with a timely and manageable appointment. Emergency treatments should be scheduled the same day or at least within 48 hours of your initial call.
Will the office file your insurance?
Will the office file claims for you, and better yet, is the surgeon in your PPO or DMO network? Going to a network surgeon will save you money.
Are there flexible payment options?
Make sure that your surgeon offers flexible payment options such as in-network insurance discounts, interest-free financing and payment by credit or debit card.
Interview your surgeon in person by going in for consultation before committing to treatment. Don’t be afraid to ask questions about the topics that concern you.
Get a personal referral

Preparing for your Appointment
Before your consultation appointment, you may want to prepare by making sure that all of the information needed by Dr Rezvani and the office team is available. Patient registration forms, health history forms and HIPAA healthcare privacy information forms are available by clicking here or by navigating to the Patient Forms and Instructions section of our site. A panoramic x-ray that is not greater than 12 months old is usually required for consultation. If you do not have one, we will be happy to take one for you when you arrive in our office. If we will be filing insurance for you, please provide all insurance information to our office team prior to your appointment.
If you will be having treatment with IV sedation, please prepare for your appointment by following these instructions:
- Please do not eat or drink anything, including water for at least 6 hours before your appointment time.
- Please bring a responsible adult with you to your appointment to drive you home. Your driver should be present in the office during your appointment and available at all times while you are in our office. Patients under the age of 18 must be accompanied by a parent or legal guardian.
- Wear comfortable, loose fitting clothing and flat rubber soled shoes.
- Take any medications prescribed as directed by your surgeon.
- Empty your bladder immediately before your appointment time.
- Get a good night’s sleep the night before your appointment.
If you will be having treatment with local anesthesia injections and/or nitrous oxide sedation, you may eat a light meal at least a couple of hours before your appointment. You may drive yourself home if you choose. Please take your medications as prescribed by your surgeon.
